Beginning January 1, 2021, all participants will be required to use the Farm Transfer Release Form for all participant-to-participant farm transfers.
To expedite on-boarding new farms, this form should be emailed to firstname.lastname@example.org for every new farm that begins shipping milk to your organization, even if the producer does not believe they have previously participated in the FARM Program. If you don't do this, and the farm already exists, the transfer will be delayed until the form has been returned.
If you need to verify that a supplier is a participant in good standing in FARM Animal Care, please request that the supplier provide a satisfaction of participation requirements letter (coop/processor only) OR evaluation certificate (farm only). A signed participation form is not an acceptable method for ensuring that a participant is enrolled, plans on enrolling, or is in good standing in FARM Animal Care.
